Qualifications

• Bachelor’s degree in social sciences or related field and 2 years of social services experience.
• Alternate: 6 years of social services experience in related disability work.
• 12 months as a Case Resource Manager Trainee or equivalent education/experience substituted.
• Ability to manage case files, assessments, and service plans with confidentiality.

Responsibilities

• Identify client needs and plan/authorize payment for services.
• Schedule home visits and conduct face-to-face reviews with clients and families.
• Facilitate person-centered goal-planning discussions and update plans.
• Complete and revise client assessments and service plans within timelines.
• Determine eligibility for programs and refer to appropriate services.
• Collaborate with community partners and state agencies; maintain thorough documentation.
