Job summary

The Commonwealth of Pennsylvania seeks a County Caseworker 1 to build rapport with consumers and their families, advocate for services, monitor delivery, and link individuals to essential supports within a hybrid remote/on-site framework.

The role focuses on connecting people with resources, keeping progress on track, and collaborating with providers. The position emphasizes crisis intervention, service planning, and continuous family-centered support in a public care setting.

Qualifications

• Two years of experience as a County Social Services Aide 3 and two years of college coursework including 12 credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences; or
• A bachelor's degree including or supplemented by 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences; or
• Any equivalent combination of experience and training with 12 credits in the listed social sciences and one year of paraprofessional case management experience; or
• Applicants will be considered to have met education requirements within two months of graduating with qualifying degree/credits.

Responsibilities

• Monitor, link, and evaluate services; link to other services and make referrals as needed.
• Provide regular direct contact and face-to-face case management while meeting productivity standards.
• Schedule and facilitate meetings to develop an individualized service plan with input from the individual and/or family.
• Enter necessary information and maintain records in state computer systems and charts according to regulations and policy.
• Offer crisis intervention services and coordinate assistance as needed.
• Collaborate with Agency staff, providers, and community resources to support individuals.
• Link individuals to transportation services or arrange transportation to appointments.

Education

Bachelor's degree with 12 credits in sociology, social welfare, psychology, gerontology, criminal justice, or related social sciences

Equivalency: 12 college credits in related social sciences plus two years of County Social Services Aide 3 or similar paraprofessional experience
