The role requires direct contact, plan development with individuals and families, data entry in state systems, and crisis coordination, with a hybrid work schedule and regular meetings.

Qualifications

• Six months of experience as County Caseworker 1
• Completion of County Social Casework Intern program
• Bachelor's degree with social welfare major
• Bachelor's degree plus 12 college credits in sociology/social welfare/psychology and one year of casework
• Equivalent combination with 12 credits in social sciences
• Within two months of graduating with qualifying degree/credits

Responsibilities

• Service Monitoring: monitor, link, and evaluate services; refer as needed
• Direct Contact: provide regular face-to-face case management
• Plan Development: schedule and facilitate meetings for individualized service plans
• Information Management: enter and maintain data in state systems and charts
• Crisis Support: offer crisis intervention and coordinate assistance
• Collaboration: work with staff, providers, and community resources
• Transportation Support: link to transportation or transport to appointments
