Job summary

Default Brand is seeking a Consumer Services Coordinator (Case Management) for Riverside School Age West to coordinate services for individuals with developmental disabilities in the Riverside West area, including driving to client sites. Mileage reimbursement is provided monthly, and the role reports to the Program Manager.

The position emphasizes person-centered planning, IPP development, and advocacy coordination with families, schools, and service providers.

Qualifications

• Bachelor’s degree from an accredited college or university and one year of experience in case management or related field.
• Bilingual preferred but not required.
• Excellent verbal and written communication skills.
• Must have a valid California driver’s license and be able to drive to consumer sites.

Responsibilities

• Coordinate case management services for individuals with developmental disabilities.
• Develop and implement IPPs, track progress, and conduct annual reviews.
• Attend IEP, ITP, IHC meetings and prepare necessary paperwork.
• Provide advocacy and link clients with community resources.
• Maintain Medicaid Waiver documentation and ensure timely completion of forms.
• Organize travel to visit consumers and families, vendors, and facilities.
• Assist in crisis intervention and document all activities promptly.
