Default Brand is seeking a Consumer Services Coordinator (Case Management) for the San Bernardino School Age unit. The role focuses on coordinating services for individuals with developmental disabilities and their families, under IPP planning and various required meetings.

Mileage reimbursement is provided for travel, and the position supports a hybrid work arrangement. The CSC will manage case coordination, advocacy, and documentation while ensuring Medicaid standards and timely reporting.

Qualifikationen

• Bachelor’s degree with one year of case management experience, or a Master’s degree from an accredited college/university.
• Bilingual preferred but not required.
• Ability to establish and maintain effective working relationships with others.
• Must have and maintain a valid California driver’s license and liability insurance.

Aufgaben

• Coordinate CSC daily responsibilities in providing case coordination services.
• Utilize Person-Centered Planning to create and execute the IPP and generate progress reports.
• Attend relevant meetings (IEP/ITP/IHC) and prepare paperwork as needed.
• Seek and utilize generic resources for consumers and their families.
• Complete documentation, milestones, and regulatory compliance activities.
