Job summary

Service Access & Management, Inc. (SAM) seeks a Service Coordinator for its Long-term Support Services team in Monroe County, PA.

You will provide ongoing service coordination for Medicaid-eligible participants, linking to community resources and developing detailed service plans to support health, safety, and wellness. The role requires compassionate, detail-oriented individuals who thrive in collaborative, community-based settings and maintain strong relationships with participants and care

Qualifications

• Three or more years in social service or healthcare settings.
• Bachelor's degree in Social Work, Psychology, or related field with practicum experience.
• Ability to work independently and manage a dynamic caseload.
• Strong interpersonal, organizational, and communication skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Ongoing assessment of participants and comprehensive assessments.
• Referral to needed/approved services.
• Develop and maintain service plans outlining strengths and needs.
• Monitor adherence to plans and participants' safety and wellbeing.
• Communicate priorities to participants and team members.
• Monitor Medicaid eligibility for assigned participants.
• Build positive relationships with participants and team members.
