Qualifications

Responsibilities

• Coordinate services for participants across Medicaid Waiver programs, MA State Plan, and other resources
• Maintain participant-centered plans, documentation, and regulatory compliance
• Work in a hybrid model combining remote work with field-based participant support
• Document accurately and timely, participate in meetings and trainings, and collaborate with interdisciplinary teams
