As a Service Coordinator within our Long-term Support Services team, you'll play a vital role in supporting participants who are Medicaid eligible under Community Health Choices waiver through providing ongoing service coordination services to eligible participants including ongoing monitoring to assure individual health, safety, and wellness; linkage to community and available resources; communicating information to participants; and developing and maintaining service plans outlining participant strengths, needs, and services . $22.00/hour, per the current union agreement

This position is part of a collective bargaining unit and follows union-negotiated terms for compensation and benefits.
This position is based in Monroe County, PA. Work is primarily community-based with some remote flexibility.
Office hours are Monday through Friday, 8:30 AM to 4:30 PM.
Serivce Coordinators have on-call responsibilities.
