Qualifications

Responsibilities

• Assist individuals with developmental disabilities to access and coordinate needed supports in community settings.
• Coordinate information for the support plan and arrange assessments.
• Assist individuals in applying for Medicaid and accessing resources.
• Maintain eligibility documentation and monitor redetermination deadlines.
• Oversee ISP participation and evaluate service effectiveness.
• Conduct monthly face-to-face contacts and document all contacts.
