The position requires a bachelor’s or graduate degree in social work or related field and at least two years of direct clinical care experience or supervised field placement in case management or behavioral health.

Qualifications

• Bachelor-level degree in social work or related field.
• 2 years full-time equivalent experience providing direct clinical care or supervision field experience in case management/behavioral health.

Responsibilities

• Accept participant assignments and fulfill responsibilities per needs and standards.
• Develop individual person-centered plans via assessments with individuals, families and agencies.
• Coordinate services per COMAR, DDA Scope of Service, and Plan of Service.
• Participate in interdisciplinary team planning and service execution.
• Advocate for participant well-being and rights at all times.
• Perform referral and intake duties as assigned.
• Maintain professionalism, dignity, and confidentiality in all services.
