Summary of position

This individual will make a difference in the lives of many by assisting people with intellectual challenges and developmental disabilities to access and coordinate needed support and services in community settings. Knowledge of social, economic, health or rehabilitative services objectives is important, as is knowledge of methods of compiling, organizing and analyzing data. This person must have strong problem-solving skills and the ability to plan and develop programs or program components. The case manager must evaluate and monitor service delivery and must possess the ability to provide technical assistance regarding program and client issues. An effective communicator, this person will be able to establish and maintain effective working relationships with others and to understand and apply applicable rules, regulations, policies and procedures.

Essential Functions
  • Assists people with intellectual challenges or developmental disabilities to access and coordinate needed support and services in community settings.
  • Coordinates and facilitates all activities related to gathering information to be included in the support plan.
    • Arranges and obtains clinically pertinent assessments.
    • Reviews clinical information and recommendations for interventions and services prior to the ISP meeting.
  • Assists the person, family or advocate to identify and access the most cost-effective services and community support based on the individual’s and family’s interests and preferences.
    • Assists individuals and families in applying for/maintaining Medicaid Eligibility.
    • Accesses community and internal agency resources (clinical professionals, etc.) in the process to identify and attain needed services.
    • Monitors annual Medicaid redetermination dates and assists the individual in completing necessary paperwork and submission.
    • Maintains necessary documentation regarding eligibility in individuals’ charts.
    • Checks the Georgia Health Partnership web site monthly to detect issues/concerns related to eligibility.
  • Oversees the person’s participation in the services, support and activities delineated within the ISP.
  • Determines the extent to which support, and services meet the needs and expectations of the individual and family.
  • Visits and meets with the individual at the individuals, families, or guardian’s request, or when there is a need to meet.
  • Makes accommodation to schedule ISP meetings in accordance with family availability as to allow for their participation.
  • Ensures at least an annual review/update that summarizes the prior year’s plan.
  • Identifies the individual’s vision of the future; functional/health status; identify preferences; capacities; interests; needs; and personal goals; relevant services; supports and activities in the ISP.
  • Makes necessary arrangements for the person’s use of services and support.
  • Maintains at least monthly face-to-face contact with the individual per DBHDD guidelines; and an additional contact working toward the achievement of the outcomes in the ISP.
    • Documents all contacts/services in the support notes and completes a monitoring tool.
    • Ensures all actions taken tie into the support plan.
    • Intervenes in a timely manner when the individual, family or advocate identifies a problem and reports the issue to the appropriate state and Columbus.
    • Is available after hours by phone to assist individuals and/or families in crisis.
    • Submits all required documentation in accordance with mandated deadlines.
    • Conducts business in a professional manner in accordance with internal policies.
  • Monitors the recertification process to ensure that annual recertification (including the DMA 7 and annual physicals) are completed and approved in a timely fashion.
  • Reviews grids and reports weekly to ensure actions to procure PCP signatures, assessments, and DMA 7 approval (within 365 days of last approval, no later than the due date, on or before the individual’s birth month).
  • Always maintain the confidentiality and integrity of the company while complying with HIPAA, DBHDD and The Columbus Organization’s Policies.
Position Requirements
  • Must have a minimum of a bachelor’s degree in a human service field, such as social work, psychology, special education, nursing, or a closely related field.
  • Must have at least one year of experience in serving persons with developmental disabilities or related disabilities in a case management or coordination role; or a Master’s Degree in a related field may substitute for the required experience.
  • Mobile Worker: This is a mobile position, meaning that regular travel to different work locations is essential. Will be exposed to weather conditions typical of the location and may be required to stand and/or sit for long periods of time.
  • Reside in the same territory they are assigned to work in; exceptions may be considered, due to business need.
  • Occasional evening and weekend work may be required as job duties demand.

This is a hybrid work setup. Local travel is required
