Job summary

Alliance Health seeks an In Reach and Engagement Specialist to support transition to community living. You will assess needs, educate members about services, and connect them with housing and community resources. The role requires strong collaboration with care teams and community partners.

Hybrid full-time position based in Smithfield, NC, with travel across Wayne County as needed.Reporting onsite for meetings is required, and experience in behavioral health is preferred.

Qualifications

• Knowledge of Medicaid basics, enhanced MHSUD and waiver benefits.
• Strong interpersonal and written/verbal communication abilities.
• Proficiency with Microsoft Office (Word, Excel, Outlook).
• Ability to organize multiple tasks and priorities.
• Detail oriented with strong diplomacy and problem-solving.

Responsibilities

• Complete assessments of interest in community inclusion and housing options.
• Engage and educate members on services and transitions to community living.
• Connect members to ongoing peer support and housing resources.
• Collaborate with care teams and external stakeholders to enhance resources.
• Maintain accurate caseloads and timely documentation.
• Ensure compliance with policies and procedures.
