Qualifications

• High school diploma or GED is required and at least two years of experience working with individuals with behavioral health needs.

Responsibilities

• Complete assessments to determine community needs and housing options, and document resolution of needs for members living in institutionalized settings.
• Engage and educate members about recovery principles, employment and education opportunities, and services covered under Medicaid/waivers; use person-centered dialogue and provide face-to-face support.
