Job summary

Froedtert Health seeks a Pharmacist for our Integrated Service Center in Wisconsin. The role emphasizes independent decision making, patient-centered medication therapy, and collaboration with the health care team to optimize outcomes.

The position is hybrid, with standard hours and a 40-hour workweek, requiring Wisconsin licensure within 6 months and relevant hospital or pharmacy experience as described above.

Qualifications

• Demonstrates proficient use of MS Word, Excel, Outlook for patient care communication.
• Active membership in the Pharmacy Society of Wisconsin (PSW) preferred.
• Active membership in ASHP or other national pharmacy organizations preferred.
• Pharmacist license in Wisconsin required or eligible to obtain within 6 months.
• Bachelor's degree in Pharmacy or Pharm.D. from an ACPE accredited school is required.

Responsibilities

• Promote safe and effective medication use considering therapy costs and benefits.
• Collaborate with healthcare team to address medication therapy issues.
• Ensure patient care aligns with the department's quality improvement plan and regulatory standards.
