Job summary

The ideal candidate will have a Pharm.D. or B.Pharm from an ACPE-accredited school, 1 year hospital experience (or 3 years other pharmacy experience), and will be licensed or eligible for Wisconsin licensure within 6 months.

Qualifications

• Minimum 1 year hospital pharmacy experience or 3 years other pharmacy experience preferred.
• Pharmacy degree from ACPE-accredited school required.
• ASHP-accredited PGY1 or specialty residency preferred.

Responsibilities

• Promote safe, effective medication use while considering therapy costs and benefits.
• Interact with healthcare team and patients to address medication therapy issues.
• Ensure patient care aligns with quality improvement plan and Joint Commission/CMS standards.
