RareMed Solutions seeks a Clinical Pharmacist to optimize patient outcomes through therapy assessment, clinical and distributive services, and collaboration with healthcare teams. Qualifications

• Doctor of Pharmacy or registered pharmacist degree from ACPE accredited school.
• Active and unrestricted PA pharmacist licensure.
• 1 year of non-commercial, specialty, or retail pharmacy experience.
• Excellent verbal and written communication skills.
• Effective time management and ability to manage multiple priorities.

Responsabilités

• Verify prescription orders for completeness and clinical appropriateness.
• Coordinate with medical providers to ensure optimal therapeutic outcomes.
• Communicate with patients about prescribed therapies to improve outcomes.
• Verify orders during fulfillment process.
• Participate in after-hours and on-call prescription fulfillment.
• Monitor inventories per policies and procedures.
• Identify and implement quality improvement opportunities in pharmacy services.
• Assist in daily data quality control activities.
• Assist in leading daily pharmacy huddles when needed.
• Supervise staff and ensure operations when on-site pharmacist is primary.
• Build and maintain collaborative internal/external relationships.
• Facilitate cold chain workflows per policies.
