Qualifications

• PA Pharmacist License (Required); NJ Pharmacist License within 90 days of hire (Required).

Responsibilities

• Designs, recommends, monitors, evaluates and documents patient-specific pharmacotherapy to optimize safe, effective, and cost-conscious medication use.
• Provides medication counseling and education to patients, caregivers, and healthcare professionals.
• Collaborates with physicians, advanced practice providers, nurses, pharmacists, and other healthcare professionals to optimize medication therapy and outcomes.
• Monitors medication therapy for appropriateness of use, dose, interactions, and adherence to evidence-based standards of care.
• Participates in the management of medical emergencies.
• Collaborates with teams to facilitate medication access and affordability for patients.
