Travis County Health and Human Services is seeking a dedicated Case Worker to join our Central office, delivering client-centered social services and eligibility determinations. You will interview clients, manage cases, coordinate with partner agencies, and advocate for vulnerable individuals and families with professionalism and empathy.

Formación

• Bachelor's degree in one of the Social or Behavioral Sciences AND two (2) years experience in social services related work.
• Any combination of education and experience that has been achieved and is equivalent to the stated education and experience and required knowledge, skills, and abilities sufficient to successfully perform the duties and responsibilities of this job.
• Valid Texas Driver's License.

Responsabilidades

• Interviews clients, determines eligibility for assistance and monitors program compliance.
• Provides case management services, develops service plans, and follows up on progress.
• Coordinates with service agencies, makes referrals, and advocates for clients.
• Assists in contract development and monitoring.
• Plans, organizes and participates in outreach, education and special activities for clients.
