Job summary

Indiana Professional Management Group (IPMG) is seeking a Case Manager for a full-time, hybrid role serving Greene County, Indiana. The position requires residing in Greene County and traveling to attend in-person meetings several times a week.

You will help individuals navigate Medicaid waivers, develop tailored support plans, and coordinate with service providers to meet goals. IPMG offers travel within the county and a strong focus on community-based services.

Qualifications

• Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or related human services.
• Or Bachelor's in an unrelated field with at least one year of direct experience with individuals with intellectual/developmental disabilities, or two years with aging population.

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process.
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve.
• Help Individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.
