Job summary

People Incorporated Mental Health Services in Anoka County, MN, is seeking a Case Manager II - Waiver Case Management to work with clients diagnosed with chronic mental illness. You will perform assessments, develop and monitor individual plans, and coordinate services while maintaining documentation and scheduling autonomy.

Full-time, Monday–Friday, 8am–4:30pm, hybrid location. Rate is $24.25–$25.75 per hour with a $1,000 external-hire bonus.

Qualifications

• Ability to perform functional assessments and develop individual plans.
• Review and adjust plans to maximize client support.
• Assess client needs and ensure continuity of care.
• Make referrals to resources based on assessment.
• Maintain daily records for continuity of service and reporting.
• Document interventions and interactions per standards.
• Attend client monthly meetings to monitor services.

Responsibilities

• Regularly review plan effectiveness and adjust to maximize support.
• Assess client needs and ensure continuity of care across services.
• Refer clients to resources based on current needs.
• Maintain records and reports for funding agencies.
