Job summary

Network180 seeks a Case Manager in Grand Rapids, MI to develop Individual Plans of Service and coordinate community-based supports. The role requires completing person-centered planning and ongoing service monitoring, with travel within Kent County as needed.

The position is 40 hours per week, Monday–Friday, 8:00 am–5:00 pm, with a hybrid work arrangement at 790 Fuller Ave NE, Grand Rapids, MI 49503. A Bachelor's in human services and relevant experience are required.

Qualifications

• Bachelor’s degree in human services or related field is required.
• Six months of full-time experience with mental health, substance use, or developmental disabilities.
• Valid driver's license and reliable transportation.
• Strong organizational and communication skills.
• Experience with person-centered planning and understanding of mental illness.

Responsibilities

• Develop Individual Plans of Service through person-centered planning.
• Provide ongoing linkage, coordination, and monitoring of services.
• Work in a hybrid schedule, office in Grand Rapids with community-based duties.
