Network180 seeks a Case Manager in Grand Rapids, MI to develop Individual Plans of Service and coordinate community-based supports. The role requires completing person-centered planning and ongoing service monitoring, with travel within Kent County as needed.

The position is 40 hours per week, Monday–Friday, 8:00 am–5:00 pm, with a hybrid work arrangement at 790 Fuller Ave NE, Grand Rapids, MI 49503. A Bachelor's in human services and relevant experience are required.

Qualifications

• Bachelor’s degree in human services or related field is required.
• Six months of full-time experience with mental health, substance use, or developmental disabilities.
• Valid driver's license and reliable transportation.
• Strong organizational and communication skills.
• Experience with person-centered planning and understanding of mental illness.

We are seekinga Case Manager who will be responsible for Person-Centered Planning to develop an Individual Plan of Service. Provide on-going linking, coordinating, and monitoring of services.

This position is 40 hours a week, Monday through Friday, 8:00 am to 5:00 pm. There are no weekends, on call, or holiday requirements for this position. The employee in this position will work ahybridschedule, working in office at 790 Fuller Ave NE Grand Rapids, Michigan 49503 and within the Kent County community.

MINIMUM QUALIFICATIONS:

• Bachelor’s degree in human services related field (for example, Social Work, Education, Psychology, Sociology).
• Six months of full-time experience working with persons with mental illness, substance abuse disorder, or intellectual or developmental disabilities (fieldwork and/or internship experience can apply)
• Possession of a valid driver’s license and access to reliable transportation for job related use
• Strong organizational skills, with experience in person-centered planning
• Demonstrated understanding of persons with severe mental illness and substance use disorders.
• Demonstrated communications skills that effectively convey information
• Demonstrated active listening, i.e. listening to what other people say and asking appropriate questions
• Demonstrated time management skills

PREFERRED QUALIFICATIONS:

• Licensed BSW
• Knowledge of trauma-informed theories, principles, and practices (includes multi-faceted understanding of concepts such as community trauma, intergenerational and historical trauma, parallel processes, and universal precautions)
• Lived experiences with mental illness/developmental disabilities/substance use disorders valued
