Job summary

JEVS is hiring a Case Coordinator for the Work Ready program in Center City Philadelphia. The role supports DPW recipients with intensive case management, counseling, and linkage to services in a hybrid, full-time capacity.

You will document interactions, protect confidentiality, and collaborate with Employment and Clinical staff to help participants achieve goals and meet Work Participation Rate targets.

Qualifications

• Experience in case management or similar social services.
• Strong communication and documentation skills.
• Ability to counsel clients and coordinate services with teams.

Responsibilities

• Provide in home and on site intensive case management support to caseload of DPW recipients.
• Match and link clients with services; make initial calls and orient clients to agencies.
• Document all contacts and maintain accurate case records.
• Maintain confidentiality of all client information in all formats.
• Develop and utilize Service Plans with Clinical Staff as primary tools.
• Provide regular individual counseling to address barriers and goals.
• Schedule clients to participate in workshops and job search activities to reach WPR targets.
