Molina Healthcare is seeking a licensed Behavioral Health Transition of Care Coordinator to support Medicaid members recently discharged from psychiatric hospitals. This hybrid role combines remote work from your home office with travel to local hospitals to meet with discharge teams and members, ensuring seamless access to services on discharge and reducing readmissions.

Applicants must hold an active Washington license or equivalent as listed and have 2+ years in health care with discharge

Formación

• Licensed clinician with active state license in practice.
• Minimum 2 years healthcare experience incl discharge planning or care management.
• Valid driver's license and transportation.
• Experience with hospital discharge planning or home health preferred.
• Familiarity with CTI or similar transition models.
• Ability to work in diverse settings.

Responsabilidades

• Follow members through a 30-day transition program from hospital admission through post-discharge transitions.
• Collaborate with discharge planners, hospitalists, outpatient providers, facility staff, and families.
• Ensure safe transitions with adequate caregiving, medical and medication oversight.
• Coordinate with care teams to arrange needed services and equipment.
• Conduct face-to-face hospital visits and home visits for high-risk members.
• Use Coleman Care Transition model post-discharge.
• Educate members on medication management, follow-up care, and warning signs.
• Perform motivational interviewing to support change.
• Identify barriers to care and coordinate solutions.
• Lead interdisciplinary care team meetings.
• Provide consultation to non-behavioral health care managers.
• Travel up to 40-50% locally as required.
