Molina Healthcare is hiring a Transition of Care Coach (BH Licensed) to coordinate discharge planning for Medicaid members in the Spokane area. This hybrid role involves travel to local hospitals to meet discharge teams and ensure members receive services post-discharge.

Required WA licenses and 2+ years in care coordination/behavioral health. Mileage reimbursement; M–F 8 AM–5 PM PST. Focus on reducing readmissions and seamless transitions.

Qualifications

• Minimum 2 years in health care with hospital discharge planning, care management or behavioral health setting.
• Licensed behavioral health clinician with WA license(s) listed in job description.
• Valid driver's license and reliable transportation for local travel.
• Knowledge of Care Transitions Intervention (CTI) or similar model preferred.
• Strong communication and organization skills; ability to work independently.

Responsabilités

• Provide transition of care support from hospital admission through 30-day post-discharge window.
• Coordinate with hospital discharge planners, hospitalists, outpatient providers and family/support network.
• Ensure safe, timely transitions with appropriate caregiving and medical oversight.
• Arrange and assess home/community services and equipment as needed for safe transitions.
• Conduct face-to-face hospital visits and post-discharge home visits for high-risk members.
• Lead and participate in interdisciplinary care team meetings (ICT).
• Educate members using transition pillars: meds, PHR, follow-up, warning signs, nutrition, functional needs, and community resources.
