Job summary

Molina Healthcare is seeking a licensed Behavioral Health Transition of Care Coordinator to support Medicaid members discharged from psychiatric hospitals. This hybrid role combines remote work from a home office with travel to local hospitals to coordinate services and resources for safe, timely transitions.

The role focuses on reducing readmissions by ensuring access to medications, follow-up care, and community supports while working with discharge planners and care teams.

Qualifications

• Must be a licensed behavioral health clinician with active license in WA state.
• Experience in hospital discharge planning, care management or behavioral health setting preferred.
• Valid driver’s license and reliable transportation for travel to hospitals.

Responsibilities

• Follow a member through a 30-day transition from hospital admission to other settings.
• Collaborate with discharge planners, hospitalists, providers, staff and family.
• Ensure safe and appropriate transitions with necessary caregiving and medical oversight.
• Coordinate with care teams and community resources to arrange services and equipment.
