Mass General Brigham is seeking a Social Worker to perform comprehensive psychosocial assessments and develop integrated treatment plans for inpatients, outpatients, and ED patients. The role emphasizes patient- and family-centered care within a multidisciplinary team, balancing medical and social needs.

Full-time position with hybrid schedule (2–3 days onsite in Lynn, MA and Danvers, MA; 2–3 days remote) and a competitive pay range.

Qualifications

• Master's Degree Social Work or relevant Master's degree required.
• Driver's License and reliable transportation required.
• Licensed Independent Clinical Social Worker (LICSW) or Licensed Clinical Social Worker (LCSW) for State of MA required.
• Must be willing to obtain MA licensure (cost covered by MGB).
• 2+ years clinical experience in a medical setting required.
• Case management experience preferred.

Responsabilités

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborate with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitor, evaluate, and record client progress according to measurable goals described in the treatment and care plan.
