Qualifications

• Master's Degree in Mental Health & Behavioral Medicine or Social Work.
• Active LICSW or LMHC licensure required.
• 2+ years clinical experience in a medical setting.

Responsibilities

• Provides psychosocial assessments of patients and families with social, emotional, interpersonal and/or environmental issues.
• Formulates biopsychosocial assessment, disposition, and treatment plans.
• Collaborates with other professionals to evaluate patients' medical or physical condition and to assess client needs as part of the interdisciplinary team.
• Monitors, evaluates, and records client progress according to measurable goals described in the care plan.
