Job summary

TRU Community Care in Lafayette is seeking a Care Coordinator to manage scheduling of appointments with specialists and community clinics for PACE participants. The role ensures CMS regulations are followed and coordinates with the PACE Medical Records office to confirm timely provider notes.

Collaborate with outside providers, participate in clinic planning, and support data sheets and charting efforts. This is a hybrid role with four in-office days and one day from home."

Qualifications

• 3+ years administrative experience in medical related field.
• Must be computer proficient and able to work with various databases, and be competent with Word, Excel and PowerPoint programs.
• Ability to collaborate with team and manage several projects simultaneously.
• Experience working with the frail or elderly population preferred.
• Experience working with individuals with mental health needs desirable.

Responsibilities

• Assist in prioritizing the scheduling of participants.
• Schedule all appointments for participants with specialists and outside medical appointments when requested by the medical team. Will create authorizations for scheduled appointments within the EMR.
• Documents in the medical record as appropriate to scope of practice.
• Perform any necessary filing or clerical duties necessary to complete charting requirements.
