Job summary

Greater Good Health in Helena, MT is seeking a Care Navigator & Medical Assistant to join our senior-focused care team. This hybrid role blends patient outreach, care coordination, and front-line MA duties to deliver proactive, value-based care in our local clinics.

You will schedule AWVs, conduct outreach, coordinate with Nurse Practitioners, retrieve medical records, and ensure timely follow-up while protecting PHI in a fast-paced, compassionate environment.

Qualifications

• Medical Assistant Certificate and 2+ years MA experience.
• Post-secondary education: 1–2 years or an Associate’s degree; a BA/BS is a plus.
• Strong verbal/written communication and empathy with patients.
• Experience with Medicare/geriatric populations is a plus.

Responsibilities

• Schedule AWVs and inbound calls for appointments.
• Outreach to geriatric patients for home, telehealth, and in-clinic visits.
• Coordinate with Nurse Practitioners for patient care and follow-up.
• Retrieve and reconcile medical records from providers.
• Post-discharge follow-up to reduce readmissions.
• Maintain PHI/HIPAA-compliant documentation.
• Assist with MA duties when clinic opens.
