Collective Health is hiring a Care Navigation Care Coordinator to guide members through the healthcare system with empathy. You will partner with care managers and clinical teams to coordinate seamless transitions and respond to benefits questions.

Ideal candidates have 2+ years in healthcare navigation, bilingual English/Spanish, and experience with EHRs and Google Workspace. Hybrid Lehi, UT role with strong collaboration and detailed record-keeping responsibilities.

Qualifications

• 2+ years in healthcare navigation, case management, or related roles.
• Professional bilingual fluency in English and Spanish.
• Working knowledge of community health resources and social determinants of health.
• Strong listening, clear communication, and daily task organization.
• Experience using electronic health records, digital databases, and Google Workspace.

Responsibilities

• Guide members by phone to navigate care options, benefits, and local resources.
• Coordinate timely care transitions with care managers and clinical staff.
• Research and resolve inquiries about plan coverage, claims, and records.
• Collaborate with internal teams and external providers to deliver coordinated support.
• Record interactions and care activities in electronic health record systems.
