• Evaluates member needs and barriers to care and facilitates the plan for outcomes.
• Develops ongoing care plans and collaborates with providers to identify resources.
• Coordinates between member, family/caregivers and care team to ensure person-centered care.
• Provides education to members and families on care options and benefits.
• Monitors member status and revises care plans as needed to meet goals.
