Qualifications

• Valid driver's license with reliable transportation and current automobile insurance.
• Experience working with seniors, individuals with disabilities, or vulnerable populations preferred.
• Experience in care coordination, case management, healthcare, social services, or related field preferred.
• Bilingual in English and Spanish preferred to better serve diverse client base.
• Strong written and verbal communication skills.
• Excellent organizational and time management abilities.
• Comfortable using computers and electronic documentation systems.

Responsibilities

• Develop, implement, and monitor individualized care plans based on client needs.
• Conduct monthly face-to-face visits and complete documentation.
• Assess client needs, identify barriers to care, and coordinate interventions.
• Collaborate with physicians, caregivers, and community partners to ensure continuity of care.
• Schedule and coordinate appointments, referrals, and services.
• Educate clients and families about resources and care plan goals.
• Monitor progress and adjust care plans as needed.
• Maintain confidential client documentation per policy and regulatory requirements.
• Advocate for clients while promoting independence and informed decision-making.
