Job summary

Qualifications

• Requires BA/BS degree in a health care related field.
• Minimum 1 year of experience working directly with people related to the specific program population or related community-based organizations.
• Travels to worksite and other locations as necessary.
• For MMP, requires Community Health Worker certification to be obtained within one year of employment.

Responsibilities

• Oversee the member's individual service plan within scope of position and contract.
• Conduct telephonic or face-to-face health assessments using predefined questions.
• Identify members with potential health care needs and coordinate with clinical care team to provide care coordination support.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective service use.
• Establish short- and long-term goals with the member, caregivers, family, and physicians.
• Identify members who would benefit from expanded services.
