Job summary

Elevance Health seeks a Care Coordinator to manage members' individual service plans per state law and contract. You will conduct telephonic or in-person assessments to identify needs and coordinate with the clinical health care management team for care coordination support.

You will also handle non-clinical needs for members with chronic illnesses, setting goals with members and families, and spotting opportunities for expanded services while traveling to worksites as needed.

Qualifications

• Requires BA/BS degree in a health-care related field; other combos acceptable by law.
• Community Health Worker certification to be obtained within one year.
• Experience with program populations and community-based organizations preferred.

Responsibilities

• Manage member's individual service plan within state law and contract.
• Perform telephonic or face-to-face history and needs assessments.
• Identify members with potential clinical health care needs and coordinate with the clinical team.
• Coordinate non-clinical supports for members with chronic illnesses.
• Establish short and long-term goals with members, caregivers and physicians.
• Identify members who would benefit from expanded services.
