Qualifications

Responsibilities

• Manage member's individual service plan within state law and contract.
• Conduct telephonic or face-to-face history and needs assessments.
• Identify members with potential clinical health care needs and coordinate with the clinical team.
• Manage non-clinical needs of members with chronic illnesses to ensure cost-effective utilization of services.
• Establish short and long term goals with the member, caregivers, family, and physicians.
• Identify members that would benefit from expanded services.
• Work schedule: Mon-Fri, 8-5 EST; 40 hours with occasional evenings or overtime.
• Must reside in Cleveland County.
