Job summary

NAMI San Diego & Imperial Counties is seeking a Family Support Navigator to work hybrid in Imperial County, assisting adults connected to the San Diego Regional Center and their families with access to services. The role emphasizes case management, barrier navigation, planning, and education to support individuals in remaining safely at home.

The successful candidate will manage an active caseload, coordinate with regional providers, and deliver culturally and linguistically appropriate services

Qualifications

• Bilingual English/Spanish required.
• Associates degree in a human services field or at least 3 years of experience supporting individuals with IDD; Bachelor's degree preferred.
• At least one year of experience supporting individuals with IDD.
• Deep knowledge of Imperial County's services, resources, and culture.
• Excellent planning, organizing, problem-solving, time management, and coordinating skills.
• Able to work with minimal direction and supervision.
• Demonstrated ability to maintain confidentiality.

Responsibilities

• Maintain and manage an active caseload.
• Complete assessments to determine client needs and develop written support plans.
• Maintain regular contact with clients.
• Connect clients and families with needed resources and assist with benefits applications.
• Document services, intakes, and visits within three business days.
• Collaborate with San Diego Regional Center, clients’ families, and other providers.
• Provide culturally and linguistically appropriate services.
