Job summary

ViaQuest Psychiatric & Behavioral Solutions is hiring a Hybrid Behavioral Health Intake Specialist (LPCC/LISW). The role offers full/part-time hours with Monday–Friday 9am–5pm or afternoons/evenings as needed.

Salary ranges from $65,000 to $75,000 based on education and experience, plus a monthly incentive program for meeting productivity goals. We support children, adolescents, adults, and specialized populations with a team of mental health professionals, including counselors, social workers,

Qualifications

• Licensed LPCC, LPCC-S, LISW, LISW-S or IMFT required.
• Medicare-eligible to provide intake and assessment services.
• 3+ years of behavioral health, case management, or related clinical experience preferred.
• Experience in SNF/LTC settings and Value-Based Care programs preferred.
• Strong communication and crisis management skills.

Responsibilities

• Provide clinical oversight.
• Perform intake assessments and psychosocial assessments.
• Support regional BHI team in SNF/LTC settings.
• Contribute to Value-Based Care initiatives.
• Lead or support interdisciplinary collaboration.
• Guide a growing behavioral health program.

Our clients are supported by a team of mental health counselors, social workers, certified nurse practitioners, case managers and psychiatrists to ensure the best outcomes for those we serve.

Responsibilities may include

• Provide clinical oversight
• Intake assessments, conduct behavioral health intakes and psychosocial assessments
• Team resource support within a regional BHI team serving skilled nursing and long-term care (SNF/LTC) settings.
• Support Value-Based Care initiatives, and serve as a clinical
• Ideal for clinicians who enjoy systems-level impact,
• Interdisciplinary collaboration, and shaping a growing behavioral health program.

Requirements for this position include

• At least 18 years of age
• must be licensed as an LPCC, LPCC-S, LISW, LISW-S or IMFT
• Master’s degree in social work, professional counseling or marriage and family therapy;
• license must be maintained during employment.
• Clinical experience treating individuals with a variety of diagnoses and presenting issues is preferred.
• Must be Medicare-eligible to provide intake and assessment services and possess a minimum of three (3) years of behavioral health, case management, or related clinical experience.
• Experience working in skilled nursing facilities, long-term care settings, geriatric populations, Value-Based Care programs, and/or Behavioral Health Integration (BHI) programs is preferred.
• Experience conducting diagnostic assessments and behavioral health intake preferred. Prior team lead, supervisory, or clinical leadership experience is also preferred.
• Must be responsible, mature and flexible; must possess excellent decision-making, time management and communication skills
• Must be able to respond calmly to crisis situations; demonstrate the ability to direct others in a confident, respectful manner and exercise good judgment in interactions with residents, families, employees, peers and regulatory agency personnel.
• Must appropriately handle confidential information.
• Must have a valid driver's license and reliable transportation.
• Bilingual language skills are a plus.
