Job summary

Centennial Mental Health Center Incorporated is seeking a Mental Health Professional to join a treatment team serving diverse clients across ages. The role combines in-person visits at Akron, CO and Yuma, CO with tele-behavioral health to provide flexible care.

You will conduct screenings, develop client-centered treatment plans, and work within a multidisciplinary team to coordinate whole-person care. Colorado licensure is required, with preference for dual licensure and multilingual

Qualifications

• Master's degree required and Colorado licensure pathway (LCSW, LPC, LMFT)
• Registered candidacy in Colorado required
• Dual licensure in Colorado as LAC/ACD preferred

Responsibilities

• Provide screening and assessment to determine client treatment needs
• Develop collaborative, client-centered treatment plans with measurable goals
• Document progress toward therapeutic goals and coordinate with a multidisciplinary team
• Deliver services in person and via tele-behavioral health
• Respond to crises and participate in clinical supervision and ongoing development
• Travel within geographic region as needed and maintain timely documentation
