Job summary

DePaul Community Resources seeks a full-time ADS Case Manager to support the Roanoke region, with a hybrid schedule and base out of the Roanoke office. The role focuses on case record documentation, referrals, and service marketing, while supporting families in the ADS program.

Remote work may be arranged with the program director. The position reports to the Director of Agency Directed Services and emphasizes teamwork, data documentation, and program growth.

Qualifications

• Associate’s degree with experience in human service programs is required.
• Knowledge of CareSmartz 360 and Medicaid waivers a plus.
• Strong written and verbal communication and organizational skills.

Responsibilities

• Create and maintain caregiver profiles and client packets in CareSmartz 360.
• Prepare paperwork for intake visits and manage approvals/denials.
• Interact with referral sources and Medicaid/DSD stakeholders.
• Assist with scheduling visits, documentation, and program improvements.
