Job Description

A Human Services Practitioner provides direct intervention, action research, training, and consultative services for/to agencies, families, and individuals with various social services programs, developmental disabilities, and/or special needs. This position requires a qualified human services professional who will provide direct assistance to clients or serve as a case manager to coordinate the delivery of services.

Responsibilities
  • Design, develop, coordinate, and conduct training and consultation to families, schools, and community service agencies regarding specific needs or problems.
  • Monitor, document, and evaluate the results of these activities.
  • Provide consultation to individuals with disabilities/problems and their families.
  • Offer on-site and phone consultation related to behavior support.
  • Collaborate with other professionals and support personnel on specific disabilities/needs.
  • Work with school district staff, families, and outside agency representatives to promote positive behavioral interventions.
  • Assist in designing and implementing training activities focused on positive behavior support.
  • Develop written action plans to provide technical and problem-solving assistance to clients.
  • Develop and co-facilitate support groups for families/caregivers and link them to local providers.
  • Deliver in-service training to educators, families, and service providers, including awareness presentations and workshops.
  • Present awareness training and activities to the public, developing and providing materials.
  • Coach, mentor, and consult with youth/students, assessing strengths and weaknesses, and develop Individual Service Plans.
  • Work with school staff to recommend solutions and disciplinary actions.
  • Draft and recommend new policies and procedures to improve program activities.
  • Conduct home visits to discuss child development, complete developmental activities, and provide intensive case management.
  • Attend community meetings and events, make presentations, and serve on community committees.
Qualifications

Requires a Bachelor's or Master's degree in Education, Special Education, Psychology, Social Work, or related fields. Bachelor's degree candidates need at least two years of applicable experience; a Master's degree may substitute for experience. Specific certifications or licensures may be required or preferred for certain areas. The position may be subject to a Level 1 or Level 2 criminal background check.
