Job summary

RHA Health Services, LLC seeks a Qualified Professional to coordinate and monitor supports for people with developmental disabilities and mental health needs. You will interview clients, supervise staff, and develop person-centered plans across residential, vocational, and day-center settings.

The role requires a four-year degree in a human service field or related area with experience, plus CPR and safety awareness. A Master’s degree is preferred and benefits are included.

• Experience interviewing and coordinating service plans.

Responsibilities

• Coordinate and monitor person-centered plans and outcomes.
• Supervise and train staff involved in residential and day programs.
• Prepare incident reports and evaluations.
• Develop and update person-centered plans through assessments and interviews.
• Engage families/guardians to obtain approvals for services.
