Job summary

The State of North Carolina - Adult Correction is seeking a Human Services Provider Prog Coord I to coordinate interdisciplinary, person-centered care for individuals with intellectual disabilities and co‑occurring disorders at Mountain View Correctional Institution. You will help develop, implement, and monitor individualized cognitive, vocational, and social development plans.

Requirements include a Bachelor’s degree in a human services field and at least one year of related experience.

Qualifications

• Documented knowledge of treatment programs and their implementation procedures.
• Knowledge of basics physiological, nutritional, sensory-motor, affective, cognitive, social, communication and adaptive development.
• Experience in developing, observing and modifying person-centered plans.

Responsibilities

• Coordinate and deliver interdisciplinary, person-centered care for individuals with disabilities.
• Negotiate and implement plans with nursing, psychology, social work and therapy staff.
• Monitor goals, quality controls, and compliance with state policies.
