Qualifications

• Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: ACEN or CCNE.
• Current, full, active, and unrestricted nursing license in a US state or territory or DC.
• Foreign education credentials must be evaluated for equivalency where applicable.

Responsabilités

• Provide patient management, care coordination, and discharge planning for inpatient and outpatient settings.
• Develop and execute care plans using patient, family, and provider information.
• Lead in implementing policies and guidelines to improve patient care.
• Offer feedback to peers to support improvement and professional development.
