Job summary

Qualifications

• Minimum of 1 year of nursing experience in behavioral health or psychiatric care.
• Experience providing community-based care to high-risk adults.

Responsibilities

• Provide patient management, care coordination, and discharge planning for inpatient and outpatient settings.
• Assist Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs.
• Develop a plan of care using information from the patient, family, and provider.
• Address psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice.
