Job summary

Rochester's Cornerstone Group seeks an experienced HUD Service Coordinator in Erie, PA to support residents and coordinate housing-related services.

The role involves building referral networks, guiding case management, and advocating for residents’ well-being within a supportive housing environment.

Qualifications

• Bachelor's degree or higher in Social Work, Psychology, Gerontology, Counseling, or related field.
• Experience supporting people with social, emotional, financial, housing, family, or community need.
• Experience with HUD housing environment preferred.
• Proficiency in Microsoft Office Suite.
• On-the-job training: 36 hours during first year, and 12 hours each subsequent year.

Responsibilities

• Explains the service coordinator program to new and existing residents.
• Identifies, assesses, selects, develops, and maintains referral relationships with local agencies.
• Links residents to service providers in the community (case management, homemaker, meals-on-wheels, transportation, counseling, visiting nurse, etc.).
• Refers concerns about lease violations to the Community Manager and assists with eviction-jeopardy cases.
• Develops case plans in coordination with community assessment services.

We're looking for an experienced and motivated HUD Service Coordinator in Erie, PA.

Compensation:

$21.00-$23.00 per hour (based on experience)

Essential Functions

• Works with new and existing residents to explain the service coordinator program.
• Identifies, assesses, selects, develops, and maintains referral relationships with local agencies.
• Refers and links residents to service providers in the general community: case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, visiting nurse, preventative health screening/wellness, and legal advocacy.
• Refers concerns about lease violations to the Community Manager and works as part of the management team when a resident is identified as being in jeopardy of eviction.
• Develops case plans in coordination with assessment services in the community.
• Monitors ongoing provisions of services from the community.
• Facilitates resident meetings, community-organizing, and social activities.
• Support residents to enhance the quality of their lives.
• Works with management in mediating conflicts between residents.
• Establishes resident communications through meeting minutes, newsletter, flyer, bulletin board, etc.
• Set up a volunteer support program with service organizations in the community.
• Performs related duties consistent with the scope and intent of the position.

Qualifications

• Bachelor's degree or higher in Social Work, Psychology, Gerontology, Counseling, or a related specialty or significant work experience relevant to the position.
• Prior experience supporting people with social, emotional, financial, housing, family, or community need
• Experience with AASC Online Software for the American Association of Service Coordinators preferred.
• Experience working in a HUD housing environment preferred.
• Basic experience with Microsoft Office Suite required.
• On-the-job training required (once hired): 36 hours during first year, and 12 hours each subsequent year.
