Job summary

Hampton Roads Community Health Center is seeking a Staff Pharmacist to provide safe, accurate, patient-centered pharmacy services within our community health setting. You will review and dispense medications, counsel patients, and collaborate with providers to promote optimal outcomes.

In this role you will ensure compliance with federal and Virginia pharmacy laws, supervise technicians in the absence of the Pharmacist in Charge, support 340B program activities, and contribute to medication

Qualifications

• PharmD or BS in Pharmacy from an accredited school.
• Current, unrestricted Virginia pharmacist license.
• Immunizations and certifications as required by policy.
• Experience in community health center settings preferred.

Job description

General Description: The Staff Pharmacist is responsible for providing safe, accurate, and patient-centered pharmacy services within the Community Health Center. The Pharmacist reviews and dispenses medications, provides medication counseling and education, identifies and resolves medication-related concerns, and collaborates with providers and other members of the healthcare team to promote optimal patient outcomes. In the absence of the PIC, the staff pharmacist is response for the supervision of the pharmacy technicians.

Essential Functions

• Reviews and dispenses medications, provides medication counseling and education.
• Identifies and resolves medication-related concerns and collaborates with providers and the healthcare team to support optimal patient outcomes.
• Ensures compliance with applicable federal and state pharmacy laws, regulatory requirements, professional standards, and organizational policies.
• Supports medication access, adherence, chronic disease management, quality improvement, and the Community Health Center’s integrated, patient-centered model of care.
• Assists the Pharmacist in-Charge with pharmacy operations, inventory management, 340B program activities, medication safety, and other required responsibilities necessary for the effective operation of the pharmacy.

Pharmacy Operations and Patient Care

• Accurately review, interpret, prepare, and dispense prescriptions in accordance with applicable laws and professional standards.
• Assess prescriptions for appropriate dosage, interactions, contraindications, allergies, duplication of therapy, and other medication-related concerns.
• Provide patients with medication counseling and education regarding proper use, adherence, side effects, storage, and precautions.
• Maintain accurate patient, prescription, medication, and pharmacy records.
• Maintain appropriate security and accountability for controlled substances and other medications.
• Support inventory management, medication ordering, receiving, storage, returns, recalls, and expiration management.
• Participate in 340B program activities and follow organizational policies designed to maintain compliance and prevent diversion or duplicate discounts.

Compliance and Professionalism

• Responsible for maintaining compliance with applicable federal and Virginia pharmacy laws and regulations, Virginia Board of Pharmacy requirements, DEA requirements, HIPAA, 340B requirements, and Community Health Center policies and procedures.
• Expected to demonstrate professional judgment, leadership, ethical conduct, confidentiality, accountability, attention to detail, and a commitment to patient safety and quality care.
• Promptly communicate significant patient-safety, regulatory, medication, controlled-substance, or operational concerns to appropriate leadership.

Knowledge, Skills and Abilities:

• Strong knowledge of pharmacy practice, medication safety, and applicable pharmacy laws and regulations.
• Knowledge of community, retail, ambulatory, or outpatient pharmacy operations, including inventory management and purchasing.
• Knowledge of controlled-substance requirements, medication security, third-party billing, reimbursement, and pharmacy technology.
• Knowledge of, or demonstrated ability to learn, FQHC and 340B requirements.
• Strong analytical, problem-solving, organizational, and project-management skills.
• Ability to develop and implement policies, procedures, workflows, and operational controls.
• Ability to analyze pharmacy performance data and identify opportunities for improvement.
• Proficiency with pharmacy technology, electronic health records, dispensing systems, and related healthcare information systems.

Education and Experience:

• Doctor of Pharmacy (PharmD) degree or Bachelor of Science in Pharmacy from an accredited school or college of pharmacy.
• Current, unrestricted license to practice pharmacy in the Commonwealth of Virginia.
• Current professional registrations and certifications required to perform assigned duties.
• Current immunizations and other health-related requirements as applicable to the position and organizational policy.

Preferred Qualifications

• Experience working in a Community Health Center, FQHC, ambulatory care, outpatient, or community pharmacy setting.
• Experience with the 340B Drug Pricing Program.
• Experience providing pharmacy services to medically underserved or vulnerable populations.
• Experience with electronic prescribing, pharmacy management systems, electronic health records, and related pharmacy technology.
• Experience with inventory management, third-party billing, PBMs, and medication purchasing.
• Previous experience working as a Staff Pharmacist in a high-volume or integrated healthcare environment.

Physical Requirements:

• Ability to stand and walk for extended periods and perform routine pharmacy duties.
• Ability to sit, bend, stoop, reach, and occasionally lift or move supplies, medications, and equipment.
• Ability to lift and carry items weighing up to approximately 25-30 pounds, as needed.
