Job summary

PARS Therapy is seeking a compassionate Physical Therapist to join our home health team serving Conroe and Grangerland, TX. This on-site role evaluates patients, plans individualized treatments, and delivers exercise, gait training, manual therapy, and balance retraining in patients' homes.

Qualifications

• Active Texas Physical Therapist license.
• Graduate degree in Physical Therapy from an accredited program.
• Home health experience preferred.
• Excellent communication and compassionate care.

Responsibilities

• Evaluate patients… assess physical abilities and limitations in home health.
• Develop and implement individualized treatment plans.
• Provide therapeutic interventions: exercise, gait training, manual therapy, balance retraining.
• Educate patients and caregivers on home exercise programs and safety techniques.
• Monitor progress and adjust plans to achieve outcomes.
• Document evaluations, sessions, and progress in digital platforms.
• Collaborate with physicians, nurses, and other therapists.
• Conduct home safety assessments and recommendations to prevent falls.
