Job summary

Integral Care seeks a Landlord Outreach Specialist (QMHP) to collaborate with interdisciplinary teams and community partners, helping homeless or at-risk individuals achieve housing stability and wellness.

The role includes housing orientations, paperwork with landlords, scheduling inspections, lease signings, and coordinating rental assistance; involves driving and community-based work as needed.

Qualifications

• QMHP with trauma-informed practice.
• Suicide Safer care knowledge and practice.
• Experience delivering evidence-based interventions or therapy.

Responsibilities

• Assist homeless or at-risk individuals with housing stability and wellness.
• Coordinate housing orientations, applications, and necessary paperwork with landlords.
• Schedule and attend housing inspections and oversee lease signings.
• Develop and maintain relationships with landlords and educate them.
• Provide services related to employment, education, medical care, nutrition, financial aid, transportation, and legal assistance.
• Deliver psychosocial rehabilitative skills training and case management as needed.
