Job summary

Agency on Aging of South Central Connecticut is seeking a Housing Coordinator to join the Money Follows the Person (MFP) Team. You will identify housing options, coordinate resources, and help clients transition from institutional care to community living.

Responsibilities include locating housing units, providing listings, arranging transportation, guiding RAP and security deposit programs, and collaborating with Transition Coordinators to ensure accessible moves.

Qualifications

• Bachelor’s degree or equivalent experience in a related field.
• Knowledge of housing resources and CT landlord/tenant regulations.
• Strong organizational and communication skills.
• Experience working with diverse populations preferred.

Responsibilities

• Identify housing units based on client needs and MFP protocols.
• Provide clients with at least two housing listings.
• Offer suburban housing options first when requested.
• Arrange transportation to view housing units.
• Guide transport resources and clarify client responsibilities.
• Facilitate access to transportation funds within budget limits.
• Support RAP & Security Deposit Guarantee Program applications.
• Collaborate with Transition Coordinators to assess housing needs and accessibility.
• Obtain landlord approval and coordinate accessibility modifications.
• Deliver RAP briefings prior to transition.
• Work with Transition Coordinators for smooth transitions to community living.
